Liability Waiver:
In consideration of acceptance of this registration by the Quinte Curling Club ("the Club"), I hereby release, waive and forever discharge "Quinte Curling Club "(the "Club"), and each of its officers, employees, members and volunteers from any and all claims, demands, damages, costs, expenses, actions and cause of action, whether in law or equity in respect of death, injury, loss or damage to myself or property, arising or to arise by reason of my particpation at the Quinte Curling Club, that has not been contributed to or occasioned by any negligent act, by omission or commission, of any of the aforesaid.
Consent to Release Information:
I hereby consent to the use of the personal information provided above by the Quinte Curling Club. I hereby consent to the use of the website application for email. I understand that this application allows the Quinte Curling Club to email me directly, however my email address will not be visible to the public unless I reply directly to their email. This information is intended for the sole use of the Quinte Curling Club and will not be shared with outside agencies.
Media Release Information:
I hereby consent to the use of event related photos posted on the Quinte Curling Club Website and realated media.
For Group (TEAM) Registrations:
By accepting the terms and conditions, I am accepting on behalf of the team.
Participants under 18 years of age:
For all players under 18 years of age, a separate consent form will be required to sign off by participant and parent/guardian/authorized agent, prior to participation in the event. This consent will be emailed to the participant email on file and is required to be returned in person day of, prior to event participation.
